1.) I understand that I am solely responsible for the drop off and the pickup of my Child from Art Strong DE activities. I agree that Art Strong DE assumes no responsibility for the supervision of my Child at times other than during the program sessions unless previously agreed upon.
2.) I hereby waive and release any right I may have or acquire to make a claim against the property of Art Strong DE or any of its members employees, or agents for monetary damages caused by injury or illness to my Child or damage to the property of my Child or myself arising from my Child’s participation in the activities and use of the facilities and property of Art Strong DE.
3.) I hereby consent to allow my Child’s picture or likeness to appear in any official documentary or advertisement of Art Strong DE in any manner incidental to their participation in the event and without compensation to me.
4.) I have carefully read this agreement and fully understand its contents.
